Why this size?
Nepal issues e-passports, and the Department of Passports captures each applicant's photo live at enrolment. That is why nobody aged 5 or over brings one.
A newborn or child under 5 cannot sit still for that photo, the Department says. So a parent brings a printed one: 35 by 45 mm, plain white, no border.

Where the pixel figure comes from
Passport offices publish millimetres, not pixels, because the photograph is printed. Turning millimetres into pixels needs a resolution, and most authorities do not name one.
This one does not, so the pixel figure above is 300 DPI, the resolution print shops work to. The millimetre measurement is the requirement; the pixel figure is our arithmetic on top of it.
The file you download records that resolution in its header, so a print dialog set to actual size uses it instead of guessing at 72 or 96 DPI and printing the photo several times too large.
That matters if you are uploading rather than printing.
An online form usually states its own pixel size or file size limit, and where an authority publishes one this page uses that instead of converting anything.

Do I need to bring a photo for a Nepal e-passport?
Not if you are 5 or over. The Department of Passports says enrolment staff take a real-time photo of you at the enrolment centre, so you submit none in advance.
The embassies say the same. Washington's answers that photographs are taken at the embassy on the day of your appointment.
What photo does a baby need for a Nepal passport?

Does a Nepal passport photo need 600 DPI?
The Department of Passports publishes no resolution. Several photo sites quote 600 DPI and cite a page on the Moscow embassy's site that no longer loads.

Can I wear jewellery, makeup or tika in the photo?
Small jewellery you wear every day, such as a small nose stud, is fine if it does not shine or cover your eyes, mouth, earlobes or eyebrows. Remove party jewellery.
The Department advises against makeup, though light everyday makeup is allowed. It has no clear rule on tika, but advises not wearing one.
Does the online form ask me to upload a photo?
No. The pre-enrollment form asks for scans of your citizenship certificate, and of your previous passport for a renewal. Each scan must be 1 MB or less.
Your photo is taken at the enrolment centre, after your form is verified and your fee is paid.
