A Spanish passport photo is 32 mm high by 26 mm wide, on a plain white background, and that size is set in Spanish law. It is smaller and a different shape from the 35 by 45 mm used across the rest of Europe, so a photo made for France or Italy is wrong here.

Why this size?
Spain is the exception in Europe and the gap is not a rounding error. 32 by 26 mm is a visibly smaller photo than the 35 by 45 mm that France, Italy and Germany all use, and a slightly wider shape as well.
A strip printed for one of those applications cannot simply be trimmed to fit, because the proportions do not match either.
It has to be made at the Spanish size from the start.
Where the pixel figure comes from
Passport offices publish millimetres, not pixels, because the photograph is printed. Turning millimetres into pixels needs a resolution, and most authorities do not name one.
This one does not, so the pixel figure above is 300 DPI, the resolution print shops work to. The millimetre measurement is the requirement; the pixel figure is our arithmetic on top of it.
That matters if you are uploading rather than printing.
An online form usually states its own pixel size or file size limit, and where an authority publishes one this page uses that instead of converting anything.

What size is a Spanish passport photo?
Article 4 of Real Decreto 896/2003 asks for a recent colour photograph of the applicant's face, 32 by 26 millimetres, on a uniform, plain white background, taken from the front. 32 is the height and 26 the width, since the photograph is a frontal portrait.
Is a 35 by 45 mm photo accepted in Spain?
The decree names one size, 32 by 26 millimetres, and states no tolerance around it, so a 35 by 45 print is not the size Spain asks for.
That is the standard size in France, Italy and Germany, which makes it an easy mistake if you have applied elsewhere in Europe before.
What background does a Spanish passport photo need?
White, described in the decree as uniform and plain. Spain agrees with Italy here and disagrees with France, which forbids a white background and asks for light blue or light grey instead.
What size is a Spanish passport photo in pixels?

Can I wear glasses in a Spanish passport photo?
The decree rules out glasses with dark lenses, and anything else worn that could prevent the person being identified. It does not ban ordinary clear glasses, but they must not stop your face being recognisable.
